DRChip Community Cardiovascular Health Initiative
This project identifies those at risk for cardiovascular disease through a series of community awareness activities and health screenings in the Danville area. DRChip staff, along with an extensive network of community partners, hosts a variety of health lifestyle training programs in schools, businesses and faith-based institutions. In addition, DRChip conducts medical screenings to identify those at risk for cardiovascular disease. Identified participants are provided with options for a care facility that meets their financial constraints and with relevant self-help educational opportunities.

Potomac Hospital/ Family Health Connection
Partners in Protecting Sight
Grant Award: $10,000
The Partners in Protecting Sight project operates out of two mobile medical units run by the Family Health Connection in Prince William County, which provides primary medical services to thousands of low-income, uninsured individuals in the area. The project screens and treats patients with diabetes and cardiovascular disease for retinopathy and glaucoma.

Richmond Area High Blood Pressure Center
Patient Education Program
Grant Award: $10,000
This project provides patient education classes about lifestyle choices and their impact on cardiovascular health. These classes focus on a number of topics including good nutrition, exercise, smoking cessation and other areas of behavior modification needed to successfully prevent, treat and control chronic diseases like heart disease and diabetes.

University of Virginia Department of Family Medicine
Patientcare Device for Asthma
Grant Award: $10,000
This project evaluated a handheld computer tool “Patientcare Device for Asthma” designed to improve clinicians’ ability to provide patient-tailored asthma care. The project will evaluate the clinical utility and acceptability of the PDA in primary care practices. The project team evaluated the impact of the software on physicians’ knowledge levels about asthma treatment, physician attitudes about the handheld format, and usefulness of the tool in the diagnosis and treatment of asthma.
